Where information is available (after directly contacting the clinics, though not all have been contacted yet) basic details have been added in italics of protocols offered at each clinic.
“Mercury free” means no amalgam restorations are ever performed – a big step for any dentist to make. Many are completely mercury free practices, others just mercury free dentists. Most will be experienced, and possibly skilled, at removing amalgams as well. Patient exposure to mercury vapour in the air will be much lower in clinics/areas where amalgams are never placed. This step also reflects the dentist’s attitude, as anyone who still places any amalgam fillings at all is not yet taking the health effects seriously (unless acting unethically). Some dentists ‘do mercury free as well as amalgam,’ which means they offer a choice. While this is far better than forcing amalgams on patients, these dentists have nevertheless wherever possible been excluded from this particular list. (Peer pressure, higher profits and underestimation of the hazards may be the most common causes, but as one dentist put it, it would be a case of ‘putting mercury into people’s mouths on odd days then taking it out again on even days’.)
Most use basic safety measures during amalgam removal (unlike most dentists not listed here, who would typically remove amalgams in a highly unsafe speedy manner, if at all), but with varying degrees of precaution. Opinions differ on what is necessary or advisable. Copious rinsing, high volume suction, cutting and sectioning (rather than drilling), and sequential removal according to the electrical readings of agalvanometer are used by most, whereas not everyone has gone to the effort to fit out their clinic with specialmercury filtration systems (preferably near the patient), nor does everybody use a nose piece with analternative air or oxygen supply. Probably the most significant safety measure of all is simply allowing lots of time (unlike in the speedy drilling of regular dentistry) for the copious water suction and rinsing, in between careful skilled cutting of the amalgam.
Only a small handful, however, excel at “holistic dentistry” with advanced testing and detoxification procedures. For some this might include, for instance, cavitation (NICO lesion) detection and surgical cleaning (possibly relevant for patients who have ever had teeth extractions or root canal therapy). Not all mercury free dentists agree, but some consider the cleaning of cavitations, and/or extraction of those root-filled teeth which test as toxic, to be as important to systemic health as the removal of amalgams, though only a few advanced and experienced mercury free dentists currently address this issue (under 10% of those listed below). Other dentists, therefore, may sometimes refer patients when needed for special examinations and/or treatments of this kind.
Not all mercury free dentists are completely metal free, which is arguably another important consideration, due to the nickel, cadmium, aluminium, palladium, beryllium, titanium and other metals used in many non-amalgam dental materials. Non-toxic completely metal-free filling materials now exist (such as the “DiamondLite” and “DiamondCrown” range) which are, moreover, more durable and stronger than amalgams.
Other considerations include the use of non-abrasive teeth-cleaning methods which do not involve scraping (note: teeth-cleaning if there are amalgams in the mouth can be hazardous in itself anyway due to the heating, which increases mercury release), and the use of anaesthetics which do not contain adrenaline. Finally, although it is normal practice to use a rubber dam during the placing of composites – which can itself require more skill and time (and often special training) compared to the placing of amalgams – the use of one to cover the surrounding teeth during amalgam removals is adamantly defended or rejected by different mercury free dentists, as there is currently much professional disagreement on whether it is effective or counter-effective due to mercury penetrating the latex and getting trapped under it. The British Society for Mercury Free Dentistry still use and recommend the rubber dam, and moreover point out that latex-free rubber dams are available now. They also claim that its effectiveness depends primarily on using it with adequate skill. Others have noted that no method can totally prevent some degree of leakage of mercury “dust” (sludge) into the throat, hence the importance of taking lots of time and rinsing constantly. TheInternational Academy of Oral Medicine and Toxicology, on the other hand, recommend a new design instead, called the clean-up aspirator tip, which fits neatly over a single tooth and, they claim, is more effective at limiting the leakage.
